The European Forum for Primary Care (EFPC) will be hosting a webinar on “Coordinated care model in Polish Primary Health Care – implementation and lessons learned”, on 5th March 2026, at 12h30-13h30 (CET), by Justyna Ledwoch, Aleksander Biesiada and Karolina Kłoda
The webinar will provide an overview of the Polish coordinated care model and will discuss its implementation (2022–2025) and evaluation by physicians and patients. Practical examples of coordinated care delivery in a PHC setting will also be presented.
